Bolton NHSFT have an exciting opportunity for a HCPC registered clinical scientist to join our friendly team which consists of 3 Haematology Consultants, a Blood Transfusion Laboratory Manager, a Transfusion Practitioner and experienced blood transfusion Healthcare Scientists and support staff. We are looking to appoint an enthusiastic colleague to provide clinical leadership to our service ensuring safe and effective transfusion practice. You will get the opportunity be involved in direct patient care, to develop a special interest and to work within a team to shape future development whilst delivering high quality transfusion care for our patients.
We provide a clinical and laboratory transfusion service to a busy district general hospital with a haematology day unit equipped for managing a full range of haematological conditions to BCSH level 2a, and a large obstetric unit.
The laboratory is well equipped (Ortho-Clinical Diagnostics Vision analysers and manual Biovue), fully computerised (Clinisys LabCentre system), fully UKAS accredited and has recently implemented a bedside transfusion administration system (Haemonetics) across the Trust,
The department is approved for HSST training and full support will be given during completion of the HSST programme and FRCPath examinations. Applications will also be considered from candidates who are in their final year of STP training.
The role involves supporting effective and safe clinical transfusion across the Trust. Key aspects of the role include:
-Providing expert advice relating to blood transfusion for direct patient care
-Providing leadership and clinical strategy for blood transfusion within the trust
-Participating in the joint obstetric and haematology clinics providing direct patient care
-Acting as governance lead for transfusion, chairing the hospital transfusion team meetings and being an integral part of the Hospital transfusion committee
-Audit lead for laboratory medicine
-Innovate and lead developments in the field of transfusion for the Trust
- Representing Bolton Transfusion at Greater Manchester Pathology network
Laboratory Medicine a Bolton is an extremely friendly and pro-active department, in which new service developments, R&D, and instilling new ways of working are part of daily working. We are an active member of the Greater Manchester Pathology network and a new LIMS system (Winpath) is due to be implemented in 2024 which will bring further opportunities to improve services.

RESPONSIBILITIES
Patient/Client Care
• Provide highly specialist clinical technical services and advice
• Undertake more complex patient cases with a higher level of patient interaction required with regard to responsibility for obtaining and assessing history and interpreting results in light of clinical information.
• Develop and implement a multidisciplinary ‘holistic’ approach to patient management, and the impact of their contribution
• Responsibility for imparting results to and discussing with patients where outcomes may be highly sensitive, emotive or have serious implications.
• Will perform the above with accountability to, and under the supervision of the Consultant Haematologist (medical lead) for Blood Transfusion.
Clinical Practice
• Requires expert knowledge and understanding of physiology and pathophysiology as applied to a broad range of clinical situations
• Provide consultant level clinical advice on a range of investigations and therapies and on complex results and their implications for patient care and management.
• Perform, interpret and accurately report arrange of complex investigations
• Direct and promote evidence-based practice and use of relevant clinical protocols
• Direct and report on relevant clinical audits
• Responsible for provision of appropriate testing strategies and development of matching clinical protocols
• Participate in direct patient care, specifically: joint haematology/obstetric clinic regarding the diagnostics and monitoring of pregnancies at risk of HDN; pre-operative transfusion planning to support the enhanced recovery programme/18 week pathway; diagnosis, investigation and reporting of suspected transfusion reactions.
Scientific & Technical Practice
• Requires expert knowledge and understanding of relevant clinical, scientific and technical principles and practice and their application to broad areas of patient care
• Provide a high personal level of scientific and professional expertise applied to critical problem solving.
• Ability to act as a source of specialist scientific advice to medical staff , allied health professionals, professional bodies, medical industry and academic institutions
• Is responsible for quality and delivery of clinical and scientific services across a broad range of patient care
• Perform and promote evidence-based practice
• Perform accurate and skilled investigations using complex and/or specialist methodology.
• Perform scientific and clinical validation of analytical results.
• Compile and authorise standard and complex reports for patients and other healthcare professionals.
• Provide result interpretation and reach conclusions on implications for patient care and management
• Provide consultant level advice in area of practice
Policy and Service
• Responsible for proposing, developing & implementing agreed policy, clinical guidelines and procedures, strategy and service developments, at departmental and corporate level.
• Ensure effective Clinical Governance of Blood Transfusion according to Trust clinical governance policies (including corporate level clinical audit of Blood Transfusion for compliance with NHSLA standards criteria for Blood Transfusion).
• Advise and provide support to the operational Transfusion Laboratory Manager (whom is responsible for Quality Management of the Transfusion Laboratory (specifically UKAS (ISO 15189)and compliance with Blood Safety & Quality Regulations 2005)).
• Ensure service complies with all relevant quality and governance procedures
• Ensure delivery of high standard of health & safety and working environment
• Ensures personal conduct at all times justifies the trust of patients and colleagues and maintains the public’s trust in the scientific profession
• Responsible for own clinical practice during diagnostic testing and therapeutic procedures.
• To challenge current practice, promoting service innovation or different ways of working.
• Building/supporting business cases for equipment/capital and staffing cost improvements
• To introduce measures to actively manage and reduce risk to patients. Ensure the formal reporting and recording of any adverse incidents, in line with Trust policy.
Financial and Physical.
• To provide expert advice to the Transfusion Laboratory Manager and other senior colleagues in determining the specification and selection of new equipment and the identification of costs associated with new equipment and techniques in Blood Transfusion.
• Provide expert advice to the Transfusion Laboratory Manager and other senior colleagues on method selection, development, optimisation and validation of equipment and methods
• To identify cost improvements within area of practice, with the aim of matching service repertoire to the needs of service users.
• Remain aware of the economic factors contribute to the formulation of consequent proposals for service development and financial initiatives.
Human Resources
• To provide leadership for Clinical Blood Transfusion to the organization
• Responsibility for the provision and monitoring of staff mandatory training and competency assessment (all staff groups including: unqualified clinical support staff, registered nurses, midwives and medical staff).
• Training, education and assessment of Medical Students for the North West Deanery
• Advise on national training requirements and curriculum for all these grades.
• Assess and make provision for the teaching and training requirements of staff, professional colleagues and users of the service
• Participate in clinical, scientific and technical teaching and training of peers, undergraduates, post graduates and other healthcare professionals within relevant areas of practice
• Actively maintains a progressive learning environment for all grades of technical staff.
• Promotes a multidisciplinary approach to education utilizing and valuing the expertise of other health care professionals and ensuring the positive contribution of the technical expertise to this process.
• Evaluate the quality of teaching and training provided and make recommendations for improvement
• May develop teaching programmes in area of specialist practice
Information Resources
• Record, store and interpret text, data and information for monitoring service activity.
• Create reports covering
o service specific statistical and management information;
o Mandatory information required by Regional & National bodies.
Research and Development
• Lead and/or participate in research and development of new tests and technologies
• Initiate or lead formal research activities (including the supervision of academic research for scientific staff studying for work-related degrees within the department)
• Direct and plan the introduction, evaluation and application of improved scientific and operational procedures to continually develop the service
• Evaluate published research and innovation and make recommendations for future service provision
• Stimulate innovation and appropriate research initiatives within the department and a wider multidisciplinary field
• Initiate and direct R&D programmes including original and translational research
• Ensure compliance with ethical and financial research governance requirements for a service
Freedom to Act
• Work autonomously to meet objectives set by senior management and clinical leads.
• Interpret and implement organisational policies and guidelines relevant to the management and development of the various services.
• As lead specialist for Blood Transfusion, will be policy/guideline author for specialist field, and will be responsible for reviewing those policies and guidelines to ensure clinical practice in the Trust is evidence-based and contemporaneous.
