About the Role:

Stronger Together (MHIDI-A) team is part of the Adult Mental Health program and is located in Thomas Street, Dandenong. We provide consultation, multidisciplinary specialist assessment and treatment to people over the age of 16 years who have been diagnosed with developmental-intellectual disability and a co-existing diagnosed mental health condition or suspected mental health condition. Many of our clients have complex presentations and challenging behaviours.

We now have an exciting opportunity available for an experienced Social Worker Grade 3 to join us, working full time hours in a permanent role.

A Day in the Life

In joining our team you would work as part of a multi-disciplinary team, to facilitate the improvement of behaviours, mental state and quality of life for clients presenting with mental illness and co-existing intellectual disability and mental illness, as well as their carers and families. You would conduct social work assessments and interventions, achieved through evidence based assessment, interventions and the utilisation of specific therapeutic and rehabilitative methods. A key requirement of this role is to educate and help build the capacity of our existing mental health clinicians and disability care professionals, as well as other relevant personnel and agencies to work more effectively with people presenting with a dual disability.

About You

You are a suitably qualified Social Worker with exceptional clinical knowledge and assessment skills, experience in capacity building and ability to work with a range of stakeholders. You will also have experience or a keen interest in the intellectual disability sector. Whilst you can work autonomously you are also a strong team player and work efficiently in a team environment and are passionate to achieve outstanding outcomes for our consumers.

Current Victorian Drivers Licence
While experience working with people with a Dual Disability, Mental Health and Intellectual Disability is highly desirable, it is not considered mandatory.

About ELMHS

About Stepping Stones Adolescent Inpatient Unit at Monash Children’s Hospital

The Monash Children’s Stepping Stones Adolescent In patient Unit is a 15 bed inpatient and five transition (Day) bed unit that offers care for adolescents aged 12 to 18 from broader ELMHS. The Adolescent Unit offers mental health Interventions to adolescents who present with acute and severe mental health problems, associated high risk factors and have difficulty maintaining their own safety and the safety of others.

This is a rare and truly exciting opportunity for a dynamic professional to lead and manage a dedicated team located in a brand new state of the art facility. We are seeking a Nurse Manager (NM) (Grade 5) to work in collaboration with the Consultant Psychiatrists to lead a dynamic, positive and cohesive team. The role requires Clinical, Leadership and Management expertise to drive an inpatient culture and model of care that delivers exceptional care to adolescence experiencing acute and severe mental health difficulties whilst offering care and support to their families / carer. This is a full-time ongoing role.

About You

As an experienced NM, you will demonstrate excellent clinical skills in working with Adolescence/Young people and Families who are in distress and face complex issues. You are dedicated to promoting their Health and Well-being through evidence-based individual and family interventions. You have extensive experience in Mental Health care and knowledge of developmental Psychiatry. You have established leadership and management skills and lead by example. Using your excellent communication skills, you work closely with a varied team of Mental Health and Allied professionals across Monash Health with the sole purpose of delivering exceptional care at all times.

You also have:

Masters in psychiatric/mental health nursing or completion of a specialist undergraduate psychiatric nursing program or specialist post basic course of training

About Us

The Casey Area Mental Health Service is led by an innovative and supportive leadership team seeking to provide an innovative and recovery based model of mental health care that will continue to develop with our growing community in the South East growth corridor. Our teams are based in both inpatient and community sites offering collaborative care to clients experiencing mental health difficulties within our area.

These roles are part of growth funding received and will initially start on site with Pakenham. Pakenham Community Mental Health Team provides an effective and comprehensive service to the Monash Adult Mental Health Program. The team work across Monday to Friday 8.30-5pm. The team provide comprehensive treatment and support to individuals experiencing complex mental health problems, at all times striving to deliver high quality recovery-focused client care as a multi discipline team. About the Role

Our Journey

The Secure Extended Care Unit (Unit 4) is a 50 bed Inpatient Unit based at Dandenong Hospital. SECUs provide medium to long-term inpatient treatment and rehabilitation for people who have unremitting and severe symptoms of mental illness or disorder with an associated significant disturbance in behaviour that preclude their living in a less restricted environment. SECU has a recovery-based model of care. It provides mental health service to patients identified as needing a longer stay in a bed based service to support extensive treatment, assessment, formulation and management recommendations.

Our collaborative multi-disciplinary team made up of: Social workers; Occupational Therapists; Clinical and Neuro Psychologist; Exercise Physiologist; Dietician; Music Therapist; Psychiatrists and Nurses provides targeted treatment and rehabilitation programs that maximise individual functioning, minimise the barriers to recovery, and promote return to community living

What you Bring

The Grade 3 Social Worker will provide quality care for clients with the use of competent clinical skills, effective communication, and active participation within the social work service and multidisciplinary teams. The role is recognised for the clinical expertise, consolidated skills and professional knowledge, ability to provide consultation and clinical leadership to health professionals within the Social Work Department and wider hospital setting, including liaison with senior medical, nursing and allied health teams.

More specifically you will be responsible for co-ordinating and leading a social work stream in the provision of effective, high-quality care for patients in the designated stream. You will also assist with management, supervision and professional development of staff and students. A caseload will be carried in designated areas, maintaining clinical expertise.

About Monash Newborn
Monash Newborn provides neonatal services to support over 9,000 births a year and is responsible for the provision of neonatal services for Monash Health within the Monash Children’s Hospital Clayton, Dandenong and Casey Hospitals & Sandringham Maternity services, functioning together as a single integrated service with a hierarchy of complexity. About 4,500 babies are born each year at Clayton to a selected high-risk maternity population, while lower risk mothers are managed at Dandenong and Casey Hospitals, which have about 2,500 births at each site each year respectively.

The neonatal nursery in Monash Children’s Hospital has a combined neonatal intensive care and special care capacity of 64 cots. Supported by a comprehensive range of paediatric specialty services at Monash Children’s, the neonatal service is able to provide the full range of medical and surgical care of the newborn with the exception only of ECMO and cardiac surgery. The volume and breadth of the service offers outstanding opportunities for clinical training and research.

About the Role
The Neonatal Fellow will work in the Neonatal Intensive Care (NICU) and Special Care Unit (SCN) of Monash Newborn and in the birthing suites and postnatal wards at Monash Medical Centre. They will work as part of a multidisciplinary team to provide safe and effective person centred care and may undertake training towards attaining Fellowship of the relevant college. The position is designed to provide the Fellow with an in-depth training in the management of a wide range of medical and surgical clinical issues in the newborn infant in a large complex multi-disciplinary health service environment while supported by Senior Medical Staff at all times.

The role includes:

Undertake advanced Neonatal clinical procedures (in line with approved scope of practice);
Participate in outpatient clinics
Active follow up role for patients
Patient assessment and management in consultation with the Neonatologist attending that area
Patient care coordination including the liaison with visiting specialists and members of the paediatric surgical team, and the management of patients under the care of their team
Attendance at high-risk deliveries
Family member communication, feedback and liaison
Provide leadership to the team of junior staff
Promote the highest standards of care through supervision and teaching of Registrars
Extensive interaction with the other health professionals, especially, the nursing and allied health staff
Ensure effective communication between all members of the nursing and medical team
Interaction with and teaching of Associate Registrars and Students
Active participation in all aspects of the Academic Program of Monash Newborn including presentation at Joint Meetings and Grand Rounds
Involvement with research is strongly encouraged and the experienced and enthusiastic support of the Senior Medical Staff will be given for the planning, conducting and reporting of their work
Play an active part in quality improvement activities
Contribute to the continuing development of Monash Newborn as a leading centre for neonatal care
Self-management (including time management, accessing education and training and completing college requirements)
Provide clinical leadership to the multidisciplinary treating team
