The Pharmacy Technician, GS-0661-07, with the Clinical Contact Center is expected to strive to achieve First Contact Resolution (FCR) whenever possible. The Registered Nurse - Polytrauma Transitional Rehabilitation Programs Admissions Coordinator will work with the interdisciplinary team for the coordination of all admissions to Polytrauma Transitional Program (PTRP), The Servicemember Transitional Advanced Rehabilitation (STAR), and the Intensive Evaluation Treatment Program (IETP) programs in Building 514 and serves as a liaison between referral sources.

• Implement treatment modalities, provide skill-based and evidenced-based psychotherapies in group and individual therapy for veterans and their families in the Trauma Recovery Services team, and as needed, the Behavioral Health Interdisciplinary Program (BHIP) teams.
• Possess a working knowledge and experience in the use of medical and mental health diagnoses, disabilities, and treatment procedures, including acute, chronic, trauma and PTSD, common medications and their effects/side effects, and medical terminology.
• Contribute to program development, consultation, and treatment planning with an interdisciplinary treatment team.
• Establish and maintain effective therapeutic relationships with veterans in Trauma Recovery Services, BHIP teams, and Community Based Outpatient Clinics (CBOCs).
• Communicate effectively, both orally and in writing with people from varied backgrounds.
• Serve as a liaison and consultant for veterans and their families regarding community resources, VA benefits, VA specialty programs, and advanced directives.

Duties include but are not limited to the following:

• Provides leadership, management, and oversight of respective Pharmacy staff and the efficient implementation
  of these services throughout the National VA facilities.
• Collaborates in the development and maintenance of written policies and procedures defining operational and
  clinical requirements for the Pharmacy program.
• Functions as a Clinical Pharmacist subject matter expert and serves as an advocate and educator on pharmacy services to various levels of leadership, including VISN and VA facilities.
• Maintains strong relationships and communication with various National and VISN levels of leadership and within NESSU and facilities served to ensure Pharmacy team functions meet expectations and proper requirements.
• In collaboration with the NESSU Pharmacy Section Chief, participates in recruitment and selection of all new staff.
• In collaboration with the NESSU Pharmacy Section Chief, conducts performance reviews, recommends appointment, special advances, within grade increases, or disciplinary action to leadership as appropriate.
• In collaboration with the NESSU Pharmacy Section Chief, establishes work schedules and assignments to ensure adequate coverage and acts upon leave requests according to VA policy.
• Promotes good labor management relations and supports Equal Opportunity program initiatives in daily activities.
• Functions as a Clinical Pharmacist for identified site facilities. Core activities include:
• Reviews patients' medication profiles and medical records as necessary for appropriate drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required. Interprets routine laboratory values and to apply that knowledge in the evaluation of drug therapy and patient care. Communicates findings with prescribers and provides appropriate alternatives to current treatment regimens as needed.
  Provides drug information to clinical staff on topics including, but not limited to clinical differences between medications, indications, proper dosages, recommended monitoring, known toxicities, necessary precautions, possible adverse drug reactions, contraindications, drug elimination routes and rates.
• Obtains medication histories from patients and summarizes significant findings in the medical record and/or to the provider as appropriate. Recommends formulary alternatives to non-VA, non-formulary medications when appropriate.
• Monitors for and reports medication errors, adverse drug reactions, allergies, and patient compliance issues. Documents findings in accordance with facility policies and procedures.
• Has expertise in the principles of clinical pharmacokinetics and pharmacodynamics and is knowledgeable and able to make dose adjustment recommendations based on objective laboratory findings.
• Participates in medication reconciliation by updating the medication profile to reflect an accurate, active list of VA and non-VA medications. Alerts provider if patient has changed his/her medication regimen to the extent that the medication management should be reviewed by the provider.
• Reviews and evaluates orders for non-formulary and restricted drugs for appropriateness and compliance with established criteria where applicable.

Duties include, but may not be limited to the following:

• Perform technical level purchasing support adhering to the Federal Acquisition Regulations (FAR), Veteran Affairs Acquisition Regulations (VAAR), Veteran Health Administration (VHA), and Prosthetics directives and handbooks, Veterans Integrated Service Networks (VISN) policy and station memorandums to acquire Veteran specific medical equipment, supplies and services based on a medical provider's clinical assessment and evaluation of the Veteran's medical condition.
• Submits Acquisition Plans and Procurement Packages for stock and veteran specific needs to the Network Contracting Office and uploads quotes, bids, invoices, consults, and other important documents to Electronic Contract Management System (eCMS).
• Verifies Data Universal Numbering System (DUNS) and Bradstreet information and ensures each vendor is in System for Award Management (SAM) and in good standing with the government to do business with.
• Verifies accountability of all prosthetics expenditures in both the purchase menu and the prosthetics program and associate all obligations to the individual veteran.
• Ensure the services provides are documented not only as the type of service, but the duration.
• Complex purchasing: requires irregular research, analysis, collaboration and many other steps to ensure both best value and the most appropriate devices are purchased for the Veterans.
• Routine purchasing: includes routine purchase orders on established contracts or through established vendor resources using regular significant research, analysis or collaboration.
• Performs other duties as assigned.
