1. Clinical responsibilities 1.1. Implement the Healthy Living Hub programme plan via the delivery of tobacco dependency interventions, providing advice and support, including advice on stop smoking pharmacotherapy and wider behaviour change support. This will involve face to face sessions in a variety of settings and venues across the Trust, working closely with community services and community pharmacists. 1.2. Support the implementation of the Ottawa Model of care in an acute setting, delivering tobacco dependency interventions and providing a support plan for each patient, including referrals, and recommending treatments based on assessed needs. Maintain Healthy Living Hub data records on a daily basis, entering all relevant patient level data, and producing and distributing monthly summary report using a range of IT applications. 2.3. On receipt of a smoking cessation referrals to the Healthy Living Hub, contact the referring physician and/or healthcare professional as required and patient promptly to arrange follow up support or referral to community stop smoking services. 2.6. Correspondence with Community service teams and other VCS partners accepting and making referrals. 2.7. Provide stop smoking support throughout pregnancy and the post-natal period, using the support of interventions such as very brief advice, Carbon Monoxide (CO) monitoring and Nicotine Replacement Therapy (NRT). Ensure that all referrals to the Stop Smoking in Pregnancy Service (SSIPS) service and Fire Service have been completed in a timely manner. Work with the Public Health Midwife to ensure areas within the hospital and community have the correct equipment and supplies and provide CO training if necessary to midwives and staff. Promote the general health and wellbeing of people who use the service by signposting them to healthy lifestyle advice and support services. Seek out opportunities to develop and implement stop smoking services in a range of maternity settings. Support the set up and on-going provision for inpatient NRT in the maternity department. As part of the South Yorkshire and Bassetlaw QUIT Programme we are increasing the support we offer to people who are admitted to, or attend our Hospitals and community based services, to help them to stop smoking or to reduce harm from tobacco and be smoke free during an inpatient admission (temporary abstinence). The programme will also provide more support to staff who wish to quit and help us to have smoke free hospitals and grounds. The QUIT programme recognises smoking as an addiction (tobacco dependency) and that it is a chronic, relapsing clinical condition that prematurely kills at least half of people who smoke. Smoking is seen as a medical condition that can be treated, rather than as a lifestyle choice. This post will contribute to tackling inequalities in health through promoting and supporting people with severe mental health problems or learning disabilities to stop smoking. The post holder will provide tobacco treatment advice and support to individuals and groups of people, who want to stop smoking, reduce their harm from tobacco or for temporary abstinence when in hospital. This will be for both patients, staff and in some instances parents/carer of people who have been admitted. Specifically the post holder will: Promote the general health and wellbeing of people who use the service through offering healthy lifestyle advice and support. Work with people to change their behavior to improve their health to include patients and staff. Act as a smoke free champion promoting the smoke free message through their day to day work.
