In line with the NHS Long Term Plan, Hertfordshire Community NHS Trust is providing clinical pharmacy services to PCNs in the local area. This is a unique opportunity for pharmacists to experience work in the rapidly expanding area of primary care, whilst retaining the support, benefits and familiarity of employment in an NHS organisation.

The Senior Pharmacist will provide expert input to the GP surgeries in a PCN, the patients they cover and locality services to achieve improvement in the quality and cost effectiveness of prescribing and achieve local and national health initiatives.

Utilising their prescribing qualification, the role will support the medication needs of the patients as well as educate other healthcare professionals on prescribing and the effective use of medicines.

The Senior Pharmacist will undertake clinical medication reviews and patient facing clinics, working in partnership with the patient’s GP practice, nurses, care homes, community pharmacists, secondary care and the local authority. · Demonstrable advanced level of clinical reasoning and judgement. Ability to apply logical and analytical skills to manage clinical risk during the use of medicines

· A full valid driving licence and access to a car to use for business purposes is essential (unless you have a disability as defined by the Equality Act 2010).

· Ability to influence other professionals

· Medication counselling skills
