Bydd deiliad y swydd yn asesu a darparu Gwasanaeth Therapi Galwedigaethol ar gyfer dinasyddion Conwy, yn benodol - oedolion a phlant ag anableddau parhaol a sylweddol.
Lle bo'n briodol byddwch yn rhoi cyngor ac yn cyfeirio os gellir cyflawni'r canlyniad mewn ffordd arall. Lle bo angen cymwys, byddwch yn llunio cynlluniau priodol, gan ddefnyddio ymyriadau creadigol ac arloesol sy’n hybu cryfderau a galluoedd yr unigolion gan gynnwys argymell offer addas, technoleg gynorthwyol, a lle bo angen cwblhau cyfeiriadau i argymell addasiadau i cartref y dinesydd, er mwyn galluogi dinasyddion i aros yn eu cartrefi eu hunain, lleihau risg a hybu annibyniaeth.
Fel rhan o'r rôl hon byddwch yn gweithio mewn partneriaeth â defnyddwyr gwasanaeth, eu gofalwyr, ac asiantaethau iechyd, gofal cymdeithasol, y trydydd sector ac asiantaethau tai. Yn ogystal â gwneud argymhellion ar gyfer offer, byddwch hefyd, yn ôl yr angen, yn ymwneud â gosod offer, arddangos offer, gan gyfarwyddo unigolion ar sut i ddefnyddio offer yn gywir fel y bo'n briodol.
Fel gwasanaeth rydym yn gweithredu dull ataliol o gyflawni canlyniadau, ac felly mae hyrwyddo'r defnydd o Teleofal a thechnoleg gynorthwyol arall yn rhan annatod o'r rôl.
Rydym yn chwilio am unigolyn deinamig gyda sgiliau trefnu rhagorol sy'n gallu gweithio mewn amgylchedd cyflym. Mae angen i chi feddu ar sgiliau cyfathrebu rhagorol a gallu gweithio'n dda gyda'r cyhoedd, cydweithwyr o fewn Iechyd a Gofal Cymdeithasol a gweithwyr proffesiynol eraill.

Work base: Coed Pella
This is an exciting opportunity to work in the Early Intervention and Prevention Team within the Conwy Disability Service in Coed Pella, Colwyn Bay.
The post holder will assist in the assessing for and provision of an Occupational Therapy Service for citizens of Conwy, specifically - adults and children with permanent and substantial disabilities.
Where appropriate you will provide advice and signposting if the outcome can be met in another way. Where there is an eligible need you will formulate appropriate plans, utilising creative and innovative interventions that promotes the individuals’ strengths and capabilities including the recommending of suitable equipment, assistive technology, and where required completing referrals to recommend adaptations to the citizen’s property, in order to enable citizens to remain in their own homes, reduce risk and promote independence.
As part of this role you will work in partnership with service users, their carers, and health, social care, third sector and housing agencies. In addition to making recommendations for equipment, you will also as required be involved in the fitting demonstrating of equipment, instructing individuals on the correct use of equipment as appropriate. As a service we implement a preventative approach to meeting outcomes, and as such promoting use of Telecare and other assistive technology is integral to the role.
We are looking for a dynamic individual with excellent organisation skills who is able to work in a fast paced environment. You need to have excellent communication skills and be able to work well with the public, colleagues within Health and Social Care and other professionals.
You will need to have good transferable IT skills to be able to navigate the various IT systems involved in recording of assessment, ordering of equipment and submission of referrals.
Relevant experience / qualifications of working with people with disabilities is required. A keen interest in / experience of community equipment including the use of technology used to support people with disabilities is advantageous.
The ability to communicate in English is essential for this post. The ability to converse at ease with customers in Welsh is desirable for this post.
Due to the nature of the work, it will be necessary to obtain a satisfactory disclosure from the Disclosure and Barring Service (DBS)
