We are seeking an experienced and enthusiastic Registered Nurse to provide exceptional care to the residents of Mt View Aged Care Facility
In collaboration with the Nurse Unit Manager and Clinical Staff the Registered Nurse will be responsible for delivering comprehensive nursing care to the Residents of the Facility.

The position is full time. Hours will be worked over a variety of shifts that are subject to regular change to suit the operations of the facility. The pay rate is as per Award (depending on number of years’ experience). There will also be a relocation incentive available for the successful applicant. RESPONSIBILITIES

The Social Worker is responsible for facilitating change and improving lives for individuals, families, community facing transitional challenges through the provision of casework and counselling.

JOB RESPONSIBILITIES:

• Conduct casework management, including gathering information, conducting psycho-social assessment, identifying common needs and goals for intervention
• Provide case work and counselling to persons with mental health conditions, dementia and their caregivers
• Plan, organise and implement community outreach activities, programmes and group work

QUALIFICATIONS

• Possess Degree in Social Work or Degree with a Graduate Diploma in Social Work from an accredited institution
