Job Summary

The Radiology Technologist may work in ambulatory or acute care environments. Performs and is competent in Radiology under the direction of the Radiologist. The Technologist must be able to independently acquire the proper exam for the Radiologist’s interpretation. The Technologist is able to operate all radiological equipment in the proper setting. The Technologist is required to work with patients in age group specific environments: pediatric, geriatric, febrile, and acute care; adjusting technique and positions for optimal examinations.

The Technologist is responsible for exam room presentation, patient transportation, entering exams into the Radiology Information System, scheduling radiology exams, educating patients on exam prep instructions, obtaining correct orders and paperwork for records, billing, and communicating with Medical Staff as requested. Technologist will obtain and maintain the knowledge needed for the PACS. The Technologist may share on call duties. Depending on the needs of the care environment may be responsible for evenings, weekends and holidays as directed by the department leader. The Technologist reports equipment malfunctions/failures to their immediate Supervisor, Maintenance staff, or Bio-med service as appropriate. The Ambulatory Case Manager (ACM) supports patient care coordination in the primary care ambulatory clinic setting. Job Description

Essential Functions:

• Optimize care coordination with hospital, ER, consulting physicians, and community resources as necessary. Collaborates with physicians, providers, and practice caregivers to identify appropriate patients for care management 
• Utilize evidence-based materials related to coordination of care and health transitions to facilitate identification of appropriate interventions for improvement and maintenance of health.
• Maintains a working knowledge and relationship with community resources and payer benefits that link the individual with the most appropriate resources. • Educates the patient and the patient’s family/caregiver about self-management tasks they can undertake to gain greater control of their health status through patient involvement in activities to improve their health, encourage wellness, avoid preventable hospital admissions, and address advance care planning where applicable. Identifies potential barriers to learning and/or care delivery and makes appropriate suggestions for change. 
• Responsible for the collaborative development and monitoring of identified outcomes measures. Conducts current and retrospective clinical reviews for the purposes of determining best practice patient care outcomes including most cost effective, appropriate care for the patient, and the effectiveness of care coordination. 
• Anticipates the needs of this patient population, seeing that necessary documentation and pre-visit planning is completed or requested before patient visit. Utilizes available resources efficiently and takes cost-effectiveness into account when assisting patients in scheduling medical services.

Job Description

Essential Functions:

• Properly prepares the examination and treatment areas per clinic standards.
• Completes patient rooming procedures to include vital signs, data collection and other identified components specific to the clinic setting and within outlined Scope of Responsibilities.
• Accurately administers and documents unit dose medications/ immunizations in accordance with clinic policy following successful training/competency.
• Performs EKG's and laboratory testing per CLIA license of the clinic following successful training/competency.
• Ensures documentation in the patient record is accurate, clear, concise within outlined Scope of Responsibilities. May function as provider scribe following successful training.
• Completes all assigned clerical activities and supply maintenance/ordering as directed.
• Assists with scheduling follow up care/appointments as ordered/directed.
• Distributes pre-printed/pre-approved patient education materials. Directs all patient questions to provider or RN.
• Functions within the documented Monument Health Scope of Responsibilities.
• All other duties as assigned.

Additional Requirements

Required:
Education - High School Diploma/GED Equivalent in General Studies; Graduate of an Accredited Medical Assistant University or accredited training facility
Certification - **Medical Assistant-Certified (MA) within 6 months of hire; Basic Life Support (BLS) Healthcare Professional Certification - American Heart Association (AHA) within 60 days of hire

** Any Medical Assistant hired  and employed with MH before July 1, 2021 has until Jan 1, 2022 to be certified as a Medical Assistant. Any Medical Assistant hired and starting employment after July 1, 2021 obtain certification with 6 months of hire.

Preferred:
Experience - 1+ years of Clinical Experience; 1+ years of Healthcare Experience

Physical Requirements: Heavy work - exerting up to 25 pounds of force constantly (67-100% of the time), and/or up to 50 pounds frequently (34-66% of the time), and/or up to 100 pounds of force occassionally (up to 33% of the time), and or up to 100 pounds of force seldomly to move objects. Possible exposure to blood, bodily fluids, or tissues. Requires the ability to work designated shift lengths (including 8 and 12 hours), which may include night shifts or occasional irregular hours.
