As a PCN Clinical pharmacist you will have an exciting opportunity to develop these new roles in practices and work within a team of clinical pharmacists supported by mentoring and supervision from a senior experienced clinical pharmacist and a GP supervisor.

Working as part of a multi-disciplinary team, you will develop and run processes for repeat prescription re-authorisation, acute prescription requests, management of medicines queries on transfer of care and systems for safer prescribing. We are looking for a CAMHS Specialist Mental Health Practitioner with CBT skills and training to join a highly-regarded MHST in Hounslow.

It is essential you hold a relevant clinical qualification and have previous experience of working in a CAMHS team. We have an exciting opportunity to join our CAMHS teams as a Recruit to Train Trainee / Trainee CBT Practitioner (1 year fixed term), as part of the NHS England (NHSE) funded Recruit to Train Programme in partnership with Anna Freud, University College London, and Kings College London.

The post-holders will be training on the Children & Young People Mental Health Therapy Course and carrying out clinical work in our service to complement their studies and use their newly developed skills. Hounslow Community Respiratory Service, supports chronic respiratory patients in the community with managing their conditions, providing them with support to Self-Manage.This job description outlines the role of a Community Respiratory Physiotherapist/Nurse responsible for managing respiratory patients and providing pulmonary rehabilitation, airway clearance, and breathlessness management clinics. It also highlights the developmental nature of the post, allowing a transition from Band 6 to Band 7, indicating that the post holder will be supported in developing skills, knowledge, and competencies to take on more advanced responsibilities over time.

To contribute to the care provision and review of the respiratory patient, thereby minimising risks and promoting recovery by supporting a multi-disciplinary approach to delivering individualised care, self-management and preventing unplanned hospital admissions.



To empower clients, adopting a problem solving approach, encouraging and giving clients the confidence and skills to improve the self-management of their chronic respiratory disease.



We are seeking a Respiratory / Oxygen Clinical Physiotherapy or nurse specialist to provide direct care for patients with Respiratory conditions in the community based Oxygen and Respiratory Case Management Team, including Pulmonary Rehabilitation and early hospital supported discharge for Hounslow residents using a multi-disciplinary team approach.



The service is to provide:

• Pulmonary Rehabilitation service; Supporting patients to manage their breathlessness through exercises and improve their functional abilities

• Home Oxygen Service – Assessment and Review (HOS-AR)

• Case Management and support of patients with unstable or more complex respiratory disease.

• Airway Clearance and breathlessness management clinic

• Primary Care Education and Support.

• Early Supported Discharge from hospital

The Hounslow Paediatric Speech and Language Therapy Team are seeking an enthusiastic and innovative therapist to lead on developing the Speech and Language Therapy Service offered to the Hounslow Youth Justice Service (YJS). This is an exciting opportunity that has arisen as a result of increased awareness in Hounslow of the Speech, Language and Communication Needs (SLCN) of young people known to the Youth Justice system and an increase in funding.

The successful candidate will need significant experience working in Youth Justice and with young people with a range of SLCN and Social, Emotional and Mental Health (SEMH) difficulties and their families.

The role includes specialist assessment and management of young people known to the Hounslow YJS within the multi-disciplinary team. It also entails developing universal and targeted input and working closely with families, peers and professionals across health, youth justice and educational settings.

As a PCN Clinical pharmacist you will have an exciting opportunity to develop these new roles in practices and work within a team of clinical pharmacists supported by mentoring and supervision from a senior experienced clinical pharmacist and a GP supervisor.

Working as part of a multi-disciplinary team, you will develop and run processes for repeat prescription re-authorisation, acute prescription requests, management of medicines queries on transfer of care and systems for safer prescribing. Also, the successful candidate will have a role in managing long-term conditions and will be expected to perform face-to-face medication reviews of patients on quality improvement and clinical audit. You will also provide primary support to general practice staff, across the PCN.

The candidate will provide proactive leadership on medicines and prescribing systems to the PCN multidisciplinary team, patients and their carers. You will also develop and deliver patient facing services and clinics which improve patients’ outcomes, reflect the needs of the PCN population and shift workload from other disciplines.

This is an excellent opportunity for an enthusiastic, motivated individual with an interest in working with children.

The Children’s Therapy Team provides services to children aged 0-19 years with varying special needs. You will be responsible for assisting the Occupational Therapy Team in a variety of community settings such as nurseries, mainstream schools, clinic setting and children’s homes.

Experience of working with children with disabilities will be an advantage, however, we do provide a comprehensive supervision structure, in-service training and peer support.

Duties will include providing general administrative and organisational support, assisting in the delivery of therapy programmes and group sessions and maintenance of prescribed equipment.
