Rehabilitation services assist patients to obtain their maximum level on independence and improving their quality of life. As a Supervisory Facility Operations Specialist, GS-1640-11, you will Personnel Management. Incumbent typically supervises the following personnel and their functions: Production Control (Construction), Production Controller, Power Production Technician, Water & Fuels Maintenance Technician, CE Supply Technician, State Superintendent, Administrative Assistant and all maintenance and state accounting personnel. Through the application of management principles, incumbent interviews applicants, selects employees, and promotes good working conditions. The Community Care (CC) Case Manager Registered Nurse (RN) is responsible for executing a streamlined approach to receiving, triaging, and directing timely, coordinated care. The nurse demonstrates proficient practice and decision-making, deliberate planning, and critical thinking skills. The CC Case Manager possesses intimate knowledge of the internal referral care process, clinical review criteria, utilization management standards, clinical documentation requirements, community care regulations, and Veterans Health Administration (VHA) standards and guidelines. The CC Case Manager uses nursing knowledge and systems theory to assess, organize, facilitate, and guide Veterans through their full range of care options including internal/direct care in Veterans Affairs (VA) and care in the community. The CC Case Manager serves as a liaison to both internal and community providers and is responsible for managing the care of the Veteran throughout the consult process. The CC Case Manager supports the medical center's mission to improve timely access to care, ensure Veterans who want to receive care in the community are referred and scheduled into the community, and support the delivery of high quality, timely care to the Veteran receiving community services.

The CC Case Manager will work collaboratively within the Office of Integrated Veteran Care (IVC) to triage Request for Services (RFS) to coordinate care processes defined by the IVC Field Guidebook to include developing a care coordination plan, reviewing medical documentation from community providers, communication of care coordination needs, and supporting the Veteran through the completion of the episode of care. The CC Case Manager will influence care outcomes by collaborating with members of the interdisciplinary team both within the Veterans Affairs (VA) and in the community. This position supports the PACT - Patient Aligned Care Team. The Clinical Social Worker possesses the skills to offer psychosocial interventions including the provision of short-term, solution-focused supportive counseling with Veterans and their family members/significant others/caregivers.

Duties may include, but are not limited to:

• Knowledgeable of the range of treatment and skills needed for all adult patients including mental health patients related to their physical limitations, psychological needs and age associated illnesses and conditions.
• Evaluate the need for Veterans to be referred for capacity exams (neuropsycholgical assessments, payee, fiduciary, etc.) through the Primary Care or Mental Health Provider.
• Identifies family/caregiver stressors and makes referrals to both VA and community resources to reduce caregiver burnout and support families and caregivers in providing care for Veterans.
• Assesses for any signs or symptoms of abuse, neglect, or exploitation and follows required local policies regarding mandatory reporting and documentation.
• Must be knowledgeable of various mental health diagnoses and understand the diversification of the mental health within the Veteran population, being sensitive to human growth and development, age-appropriate needs and concerns.
• Screen and assess for suicide risk to provide vital access to services for Veterans who may be experiencing suicidal ideations and be in need of care.
• Provides case management and care coordination to facilitate appropriate delivery of health care services, incorporating complex multiple causation in differential diagnosis and treatment of Veterans and making psychosocial and/or mental health diagnoses within approved clinical privileges or scope of practice.
• Knowledgeable regarding the continuum of care and is able to evaluate required level

Major Duties Include (but not limited to):

• Generate memos, composing, and editing letters, reviewing correspondence for accuracy and completeness.
• Assist with folding of patient/provider letters and ensure that they are mailed timely.

• The Advanced Medical Support Assistant (AMSA) position serves as a member of the Community Care Consult team. The AMSA works collaboratively with other members of expanded health care teams (i.e. PACT, specialty care) including Community providers, pharmacists, social workers, dieticians, behavioral health staff, etc. to provide a robust interdisciplinary approach to care. Important interfaces and collaborations also occur with larger health care system providers including VA and Community specialists, hospitals, diagnostic and treating facilities, and community based programs.
• The AMSA works collaboratively in an interdisciplinary coordinated care delivery model (i.e. PACT, Specialty Clinics, C&P, Community Care) and performs receptionist duties, customer service and other duties assigned for the proper and timely treatment of patients and maintains appointment schedules for all Community Care.
• Communicates with Community Care providers to ensure they accept VA patients and payments, prepare any required pre-appointment documents, schedules the appropriate level of care and Community Care Advanced Medical Support Assistant Functional Statement documents the schedule in the appropriate consult. Obtains medical documentation from Community providers, scans to appropriate consult within CPRS and view alerts the VA providers for continuity of care. Coordinates internal and external care between customers and providers.
• Enters and edits Community Care program authorizations in the Fee Basis Computer System and mails/faxes letters to vendors and patients.
• Incumbent schedules community clinic appointment using VistA scheduling options and linking appointment to consult. Complete verification with Community providers that patient made scheduled appointment.
• Meets the needs of customers while supporting VA missions. Consistently communicates and treats customers (veterans, their representatives Community Care Advanced Medical Support Assistant Functional Statement visitors, staff, and internal and external providers) in a courteous, tactful, and respectful manner.

The primary purpose of this position is: To perform as a Management and Program Analyst and subject matter expert (SME), to conduct a wide variety of studies, analyses, evaluations, planning, programming, budgeting, and execution management for the formulation, presentation, evaluation, coordination, and justification of USSOCOM programs and requirements resourcing, for development of the USSOCOM Program Objective Memorandum (POM).
