Duties include but are not limited to the following:
- Supportive individual counseling to Caregivers; Caregiver/Veteran couple's counseling when clinically indicated.
- Caregiver support groups; these may occur at the facility and/or in the community
- Coordination of Care and brief Case Management to assist Veterans in accessing services in a timely manner; this may include direct referrals to VA medical and mental health treatment services and to other VA and Community services (i.e., Home-Based Primary Care, Respite Program, etc.) including VA benefits, as well as to community-based services (i.e., Hospice) and other non-VA entitlement programs.
- Clinical triage and crisis intervention when clinically required; this includes completing all aspects of VA required risk assessments and safety planning
- Caregiver education workshops and training to Caregivers, VA staff and community partners regarding Caregiver issues
- Community Outreach at both community (Veteran Service Organizations) and VA-sponsored events (ie., Mental Health Summit) regarding VA caregiver benefits, programs and services.
- Quality-improvement activities and evaluation/consultation with Social Work Supervisor and Caregiver Support Team.
- Provide coverage for and assist, when needed, the Program of Comprehensive Assistance for Family Caregivers (PCAFC).
- Collaborate with the VA Patient Aligned Care Team (PACT), mental health and primary care clinicians to provide direct assistance to Caregivers and Veterans and related staff education on caregiver issues (ie., geriatric and Dementia care).
- Serves as a consultant for information on understanding various Caregiver/Veteran services and benefits and when negotiating the complex processes caregivers, Veterans and families encounter when interacting with government and community agencies.
- Active member of the Caregiver Advisory Board and serves as a liaison between the VA and community agencies; this includes participating in caregiver support activities in the local community and state to promote and recognize the role of the caregiver.
- Supports the planning and implementation of the events and programs associated with annual National Family Caregivers Month every November; that includes announcement and advertising of month-long activities, mobilization of participants, educational sessions, caregiver recognition events and caregiver support programs.
- Maintain a knowledge base of current theory and treatment modalities by reading professional journals, attending conferences, clinic seminars and teleconferences, and materials distributed through VA and private sector sources.
Work Schedule: Monday-Friday, 8:00 am to 4:30 pm
