This position functions in the clinical associate role in the primary care clinic, which utilizes a Patient Aligned Care Team (PACT) model. The PACT ICT is responsible for providing direct and/or indirect patient care to an assigned group of patients. She/he will have the knowledge, skill and experience to carry out assignments involving point of care testing, providing treatments and vaccinations, performing diagnostic studies, and patient/family education. The ICT demonstrates active collaboration and communication with Patient Aligned Care Teamlet (Patient, Primary Care Provider, RN Care Manager and Administrative Associate) and Larger Team (family/caregiver, internal and community-based services and disciplines involved in providing care to the patient). This is the full performance level for the ICT position. This is a bargaining unit position.

Major duties include:

• The incumbent assists outpatient licensed staff with the delivery of care to patients with complex, specialized, and potentially life-threatening situations.
• Performs observation-based screening and collecting patient information in collaboration with licensed personnel.
• Completes approved templates and tools, such as the outpatient intake form, normal result notes, and follow up treatment protocols established by the agency.
• Assists in behavioral/psychiatric situations in an outpatient setting. This can include assisting licensed personnel with crisis intervention, management of aggressive or suicidal psychiatric patients, by providing one-to-one observations and applying prevention and management of disruptive behavior techniques (PMDB).
• Performs duties focused on communication techniques required to complete the questioners and templates approved for use in any outpatient clinical setting, including active listening, restating, and mirroring.
• Maintains a safe and therapeutic environment for the patient while promoting a sense of worth and dignity under the supervision of the licensed personnel.
• Assists outpatient licensed staff with the delivery of care to patients with complex, specialized, and potentially life-threatening situations.
• Assists licensed staff with preparing patients for medical evaluation, setting up equipment, passing instruments and providing treatment during in person or virtual visits.
• Operates specialized medical equipment to obtain diagnostic and therapeutic outcomes.
• Collects a wide range of specimens and preparing them for transfer to processing.
• Observes patient status and keeps licensed clinical staff informed of any change in condition.

We are looking to recruit Facilities Assistant South (Periodic) people to work on the Tameside General Hospital / Stepping Hill Hospital sites and Community Clinics in Tameside / Stockport Localities.

Monday – Sunday working on a 5 over 7 rota – 7.5 hours per day = 37.5 hours per week, working hours to be arranged at the interview.

You will be based at the Tameside Hospital or Stepping Hill site. and will work across the Trust’s in-patient and/or community settings as assigned, which will include working on wards, clinics or admin areas.

The postholder will be a member of the Trust’s Facilities team and will provide a comprehensive cleaning service for Pennine Care NHS Foundation Trust (PCFT). which includes Periodic and Heavy Duty cleaning. The postholder will carry out periodic cleaning tasks in line with the annual periodic cleaning schedule, in order to meet the requirements of the National Standards of Healthcare Cleanliness 2021. The postholder will work as part of a team to ensure that the standards are achieved. The postholder will be required to drive a Trust vehicle in this role, or the use of a car or access to a means of mobility to travel across the Trust Footprint in line with service needs, which will include moving and collecting cleaning stores, supplies and equipment as required.

Carrying out tasks in line with Trust policy, national cleaning standards and local instruction, which will include:
• Carpet cleaning

• Curtain changing

The Role
Hallfield School is seeking to appoint a skilled Estates & Maintenance Assistant to assist with day to day maintenance and compliance of the School estate. NVQ Level 2 or above in any trade such as carpentry, plastering, plumbing or construction would also be advantageous. In addition, any experience in multi-trade skills would be highly desirable.
The role offers a varied day, within a busy environment, covering maintenance, grounds work and compliance checks and testing. It would suit an individual who takes pride in their work, is able to show initiative whilst working supportively and flexibly within a team.
Key responsibilities of the Estates & Maintenance Assistant include, but are not limited to:
o Carrying out routine maintenance of school facilities in accordance with schedules or in response to break downs including carpentry; plumbing; basic electrical; decorating; drains/gutters.
o Assisting the Estates & Maintenance Manager with the running of heating systems.
o Carrying out safety checks on all school vehicles.
o Assisting with moving furniture and equipment for school events and carry out car park duties as required.
o Responding to emergency breakdowns/incidents and provide relief cleaning in emergencies if required.
o Undertaking weekly and monthly compliance tests including statutory testing of fire safety systems and equipment tests, water flushing, emergency lighting.
o Carrying out audits and checks as requested by the Director of Estates & Facilities.

The Person
The key skills, experience and qualities required to be the Estates & Maintenance Assistant:
o Experience of working in a similar role, ideally within an educational setting
o Understanding of Health & Safety requirements and principles in a school environment including COSHH
o Knowledge of emergency procedures such as fire evacuation
o Experience of the building trade with a wide range of skills for example plumbing, basic electrics, plastering and general building work (with a qualification such as City & Guilds and NVQ being advantageous) and willingness to work across all disciplines
o Competent with a wide range of tools and machinery relevant to the role
o Effective written and verbal communication skills
o Strong literacy and numeracy skills with administrative experience for example in stock taking and report writing
o Organised and flexible, with a willingness to work unsociable hours if required
o A full driving licence is essential
