MAIN RESPONSIBILITIES Clinical To be professional and legally accountable for all work undertaken. To formulate, evaluate and reassess individualised management and / or treatment plans, using clinical reasoning and utilising a wide range of treatment skills and options in order to formulate a personalisedcare plan. Tocomplete assessments and interventions in a variety of settings utilising a range of methods to do this e.g., face to face and virtual, the clients home and in community settings. To be able to manage a caseload of people with complex health and social care needs. To provide specialist advice for clients, their families and colleaguesbased on up-to-date theoretical knowledge and relevant practical experience, and provide specialist advice and guidance to MDT colleagues, junior team members and other health care professionals both within and outside the trust. To use collaboration and partnership to empower patients to effectively self-manage their condition and identify those that may need alternative support to achieve this. CLINICAL To be professionally and legally accountable for all work undertaken. To work within the standards and guidelines of the team, the Trust, BAOT, HPC AND COT code of ethics. To independently manage a caseload of complex cases, providing specialist occupational therapy assessment and interventions to clients with complex physical, cognitive and psycho-social needs arising from their medical conditions. To provide this service in the most appropriate setting to the client, e.g. clients homes, workplace and other community settings, working closely with the client, his/her family/carer and other professionals in doing this. To be responsible for prioritising the occupational therapy referrals within the service to ensure that clients with urgent needs are seen within appropriate timescales. To establish excellent therapeutic relationships with clients/family/carers to gain their participation in the therapeutic process by employing advanced interpersonal skills (e.g. communication, reasoning, negotiation skills, motivational tactics) and overcoming any barriers to communication (e.g. different languages, lack of understanding of the spoken word, visual or hearing impairments, fear, anxiety). To assess for and diagnose any functional, physical, cognitive, perceptual, psychological and sensory deficits using functional assessments together with specialised standardised and non-standardised assessment batteries. Based on the analysis of the assessment and a risk analysis, to determine occupational therapy aims and treatment goals as part of the overall care plan, and then to formulate intervention options which reduce the impact of disability. As a Specialist Community Public Health Nurse (SCPHN) School Nurse, the post holder will work within the School Health and Wellbeing Service (SHWS) as part of the Tower Hamlets GP Care Group integrated 0-19 service to deliver the Healthy Child Programme 5-19. The post holder will provide a comprehensive service to meet the needs of children, young people and their families, applying the principles of positive health and wellbeing and the prevention of ill health with individuals, families, schools and the wider community. To formulate, evaluate and reassess individualised management and / or treatment plans, using clinical reasoning and utilising a wide range of treatment skills and options in order to formulate a personalisedcare plan. Tocomplete assessments and interventions in a variety of settings utilising a range of methods to do this e.g., face to face and virtual, the clients home and in community settings. To be able to manage a caseload of people with complex health and social care needs. To provide specialist advice for clients, their families and colleaguesbased on up-to-date theoretical knowledge and relevant practical experience, and provide specialist advice and guidance to MDT colleagues, junior team members and other health care professionals both within and outside the trust. To use collaboration and partnership to empower patients to effectively self-manage their condition and identify those that may need alternative support to achieve this. To be able to identify when a client can be discharged from the service, taking into account their confidence and competence to self-manage and to be able to effectively communicate this to the client and their support network. Leadership To support, supervise and appraise the work of supervised staff, students, and rehabilitation assistants / care navigators and to induct new staff as required. To participate in the Trust Appraisal Scheme, both as appraisee and as an appraiser. To assist and support the Clinical Leads andservice managers, taking an active part in patient engagement, audit and project leading when required. To be involved in the recruitment, selection, and induction process for staff in the MACCT / Proactive care service.
