Looking for a Community role in Adult Mental Health, to use your OT skill set?
We have an exciting opportunity for a Band 5 OT on our Reablement and Engagement Team (CORE). Working alongside OT's and Vocational Specialists within a MDTsetting. Working at in Central Croydon, easily accessible by public transport.
CORE has 2 intervention Pathways:
1. Reablement - working with clients referred by Primary Care staff. Helping develop resilience and reducing dependence on mental health services. Using, individualised goal setting and group work to support Activities of Daily Living (ADL), while also encouraging engagement with social or community options.
2. Vocational Pathway - Providing assessments and person centred support for sessions. This Pathway is delivered on a co-working basis with CMHT staff. Vocational Support is for clients who would like to engaged in social or community activities, such as training, education, volunteering and more... as part of their recovery.
You will be part of an established SLaM Croydon OT network and Peer Group. Individual OT supervision will also be provided to identify and support regular training, AHP Preceptorship and CiPD.
Applicants need to show an understanding of the importance of, and commitment to, client centred practice, They must have good communication and organisational skills, and confidence and skills in delivering both individual and group interventions. They must be able to work effectively as part of a team.
JOB PURPOSE
· To manage a caseload of clients with reablement and vocational needs, using evidence based / client centred principles to assess, plan, implement and evaluate interventions in health, community and home settings.
· To routinely provide individual and group interventions appropriate to the clients on the caseload which support their recovery goals, in collaboration with other professionals in the team.
· To liaise with Croydon Social services departments and other external agencies including GPs to facilitate clients access, support of needs, physical disabilities and discharge planning.
· To provide clinical support to the vocational practitioners in the CORE team.
· To participate in the planning, development and evaluation of Community OT / CORE Team services, holding responsibility for defined projects and activities.
·To regularly supervise OT students on practice placements and other junior OT staff (as required)
· To carry out MDT activities e.g. generic assessments and duty responsibilities.
· To work in collaboration with care co-ordinators in other community teams in delivery of Care Plans for individuals under CPA.
The South London & Maudsley NHS Foundation Trust provides comprehensive, complexmental health services both locally and nationally. Directorates are structures which bring together clinical services,education and training and academic activities within a series of single managerial units.
The occupational therapy workforce and associated Allied Health Professions workforceconsists of approximately 200 staff based in all the Trust Directorates.
The Croydon Community Mental Health teams provide multidisciplinary support and treatment to clients with a range of mental health disorders. Many service users also experience social care and physical health needs. A range of interventions are offered including; Care Co-ordination, emotional regulation, CBT, DBT, OT, Psychology, social work, medical and nursing specific assessment and interventions.
The CORE Team provides both short term Reablement Interventions (goal setting based) to promote resilience, and vocational specific interventions to support social inclusion and occupational and employment opportunities.
Key Responsibilities:
1) To undertake occupational therapy assessments for a designated caseload, assessinghealth and well-being from an occupational perspective. To contribute to assessing andcompleting risk assessments within the multi-disciplinary team to support a personsrecovery. To hold an up to date knowledge of a range of occupational therapy standardisedassessments, the models on which they are based and how to carry these out. To identifyoccupationally focused goals taking into consideration the mental, physical, spiritual,psychological and social needs of the service user and that it is sensitive to age, culture,race, gender, ethnicity, social class, sexuality and disability.
2) In collaboration with the service user, to plan and implement individual or grouptherapeutic interventions using graded activity to achieve therapeutic goals which supporttheir recovery. To use outcome measures that give clear evidence that occupational therapy has made a difference to peoples occupations and ultimately support social inclusion and promote recovery. To hold an up to date knowledge of safeguarding principlesand know how to respond to any safeguarding concern related to the service. To display anunderstanding of the potential for distress caused by, and social and personal consequences of mental illness for the service user, their families, carers and supportnetworks.
3) To interact with service users, families, carers and support networks in an empatheticfashion and demonstrate a clear understanding of the effects of disability in the areas ofproductivity, leisure and activities of daily living. To apply a high level of understanding ofthe effect of disability and provide training and advice on lifestyle changes and adaptationsto the service users social and physical environment. To assess and prescribe small aidsand equipment and recommend adaptations to maximise independence for service users inhome and community environments, where appropriate. To analyse, grade and adaptindividual and group activities to meet the occupational performance and participation,environmental and vocational needs of service users.
4) To routinely implement occupationally focused pathways that clearly identify whatservice users can expect from their occupational therapy interventions whilst workingcollaboratively with individuals to ensure treatment is targeted to their specific needs. Toensure all occupational therapy equipment and materials are maintained and storedresponsibly and advise on resources needed to carry out group and individual treatmentsincluding financial.
5) To develop a comprehensive set of skills to effectively communicate with service usersand their family using a range of communication and engagement skills. To actively promote relationships with colleagues that support open communication and collaborativeworking, taking personal responsibility for ones contribution to the process of multi-disciplinary team working. To articulate the meaning and value of occupation in relationshipto well-being, recovery and social inclusion to service users, carers and colleagues and theoccupational therapy contribution to this. To ensure that up to date written and electronicrecords and activity data are maintained in accordance with Professional and Truststandards/policies.
6) To use electronic recording systems to document high quality interventions thatenhance occupational performance. To participate in local and corporate requirements ofdata collection. To provide occupational performance data or other reports as required.
7) To participate in service and occupational therapy evaluations, clinical pathwaysdevelopment and implementation, benchmarking and audit activities and to contribute toimplementation of service modernisation and improvement projects as required. To activelyparticipate in the delivery of the Trust Occupational Therapy Strategy and via the specificoccupational therapy implementation plan in the relevant Directorates. Todemonstrate an understanding and application of national guidance and legislation relatingto Mental Health service provision in the context of well-being, recovery and socialinclusion. To be able to evaluate current occupationally focused research and to basepractice on best available evidence. To contribute to the occupational therapy research
agenda by participating in local audit and research projects.
8) To have an understanding of and demonstrate skill in the supervision process throughones own supervision and in the provision of supervision to staff and students. To ensurefull engagement in the Preceptorship programme. To represent and act as an ambassadorand role model for the Occupational Therapy profession. To participate in the induction,training and education of students and other staff.
9) To develop leadership skills, contributing to decision making about practice usingtraining and development opportunities provided by the Trust. To adhere to the COT Codeof Ethics and Professional Conduct and national and local policies. To demonstrate ongoingpersonal development through participation in internal and external developmentopportunities and recording CPD in a professional portfolio fulfilling HPC and KSFrequirements including mandatory training. To demonstrate an awareness of the effects ofand ones own limitations in fulfilling some aspects of the role on the mental well-being ofoneself and demonstrate ability to seek assistance and guidance from colleagues.
10) To demonstrate self-awareness and an ability to reflect and discuss emotions andthoughts in relation to care of service users seeking assistance and guidance fromcolleagues as necessary. To demonstrate an understanding of the benefits and challengesof working in a multi-professional team and be able to maintain a clear occupational focusto ones work for the benefit of the service user. To exercise and thus, actively role model,good time management, punctuality and consistent reliable attendance.
