The paediatric services at The Hillingdon Hospital consists of a 24-bedded general medical and surgical ward, which includes a level 1 POSCU, 5-bedded paediatric day care unit, a four bedded day surgical ward, and a paediatric outpatients department. Paediatric services at Hillingdon also included of our award winning, dedicated paediatric diabetes team; paediatric respiratory and allergy services; paediatric oncology and paediatric epilepsy service and level 1 HDU service. We also have close links with our paediatric community nursing team; paediatric emergency department; PATCH service, neonatal unit and child development centre.

An Exciting opportunity has arisen within the Frailty Team at Hillingdon Hospital.

We are seeking to expand the frailty team in line with the planned redevelopment of the main hospital site.

The frailty team cover ED (emergency department) and the short stay unit as well as rapid access clinics with the aim to reduce hospital admissions.

Our staff work as part of an innovative interdisciplinary team embedded in service improvement and dedicated to developing skills including advanced practice.

We currently run a 5 day service with aims to expand into 7 day working.

We are looking for clinicians who have passion for frailty and the provision of therapy within acute medicine and same day emergency care.

Successful applicants will have prior experience in either emergency care or acute medicine with a frailty focus. They will also show enthusiasm in working within a broad range of clinical conditions, demonstrate excellent communication skills, flexibility and enjoy both teaching and learning.

We work in operationally integrated therapy teams, and you would be expected to provide leadership for the frailty team in conjunction with other senior therapists, Therapy Team Leads and Therapy Manager in continuously developing aspects of the therapy service.

Our Frailty team took part in the Acute Frailty Network when our frailty services were first launched within the ED. The work carried out in ED saw an 18% reduction in the number of frail older adults needing an admission and a saving of 10 to 13 bed days a year. A benchmarking event saw Hillingdon Hospitals Frailty service outperform others across North West London with regards to short length of stay, geriatrician support and the number of hours Frailty service is available to patients, with aims to improve in other areas including 7 day working and increasing bed base.
• To provide highly specialised Therapy assessment and treatment for patients within Frailty, predominantly within short-stay patient areas including Frailty Assessment Unit, Emergency Department, Clinical Decisions Unit, RACE clinic and Acute medical unit

The post holder will be part of a growing leadership team which we are developing to reinvigorate our Acute Medicine and Same Day Emergency Care (SDEC) services. The post holder will join a supportive team which has a strong culture of teaching and training, additionally the Hillingdon AMU has just come top in the country for junior doctor feedback in the latest GMC Survey, which is reflective of the progressive style the team are creating.

This post will work across Acute Internal Medicine, with a focus on the Acute Medical Unit (AMU), SDEC and links with our emergency medicine department.
The post is advertised as 10 PAs. There is a possibility of additional PAs in discussion with the Divisional Director for Unplanned Care. Further subspecialty interests including specialty clinics and service development will also be welcomed and accommodated within the job plan, in addition to the Medical Specialties mentioned above.

There will also be an opportunity for the desirable candidate to be supported with a CESR/ portfolio pathway application towards the specialist register, this can be discussed further during the interview.


The post-holder will be responsible to the Divisional Director of Unplanned Care at The Hillingdon Hospitals NHS Foundation Trust. This is a 10 PA post, 8.5 PA DCC, 1.5 SPA. SPA may include clinical and educational supervision, audit, personal CPD and mandatory training. Personal CPD must be included as part of annual appraisal which is mandatory and required for the revalidation process.

The post holder will be key to developing and leading changes to our services which we are developing to better support inpatient care, patient flow, early supportive discharges, with education and training. You will provide psychiatric input to intellectually disabled adults registered with Hillingdon GPs. • To provide a Consultant Psychiatric service for people with a Learning Disability who may be admitted on Frays and Crane Ward, acute inpatient mental health wards at The Riverside Centre (2 beds). This would include providing Responsible Clinician cover, attending and providing Responsible Clinician input at weekly ward rounds, multidisciplinary meetings, administration and preparation of tribunal reports.
• To lead the collaboration with mainstream mental health services within the Borough.
• To be an active participant in the shaping of new services in light of recent and ongoing changes to the NHS, in collaboration with relevant Clinical and Service Managers.
